Universal design is simply designing to suit the needs of all stages of life.  This includes adults and children, people with disabilities and those who are aging.  A “Home Eval” (a Discharge Plan team member goes to your home and documents how well your home accommodates your present, recovering, condition) is common for age 60 plus.  Conventional  homes so seldom meet criteria that doctors order simultaneous eval of you, by a nursing home, or assisted living/rehab center.  Often, only hours before you are ushered out of the hospital, you find you are not going to your own home to recover. Thereafter, the possibility of never going home increases as the days and weeks go by.
